  • Reports must include all essential components and relevant information.
  • Discrepancies must be brought to the attention of the dictator or designate.
  • Reports must have proper spelling, grammar, format and content.
  • Reports must be transcribed to accurately reflect the dictator’s intended meaning, but may be interpreted and edited as necessary for clarification.
  • Reports are timely in their completion and submission.

Two Level Review Process

Following transcription, our QA Level 1 analysts review 100% of all files prior to return to the client. If the analyst deems that the report passes all necessary quality criteria, the document is then returned to the client. If the document is not suitable for delivery to the client, it will first be routed to a QA Level 2 analyst for a complete review with the dictator’s audio file.

Our North American Workforce

Another way that MedGarde ensures quality is by performing all transcription in the United States and Canada. Clients wishing that dictation not leave its country of origin may make that request and we will honor it in writing. No work is outsourced to places like India, Pakistan or the Philippines. Likewise, no shortcuts are taken by having dictation run through unmonitored speech recognition engines. Clients can rest assured that they are receiving their work in final form the first time.

Transcriptionist Screening & Selection

MedGarde is staffed with teams of carefully selected medical transcriptionists that create your documents with a passion for quality.  Our transcriptionists are recruited from our industry’s best educational institutions. Additionally, our staff includes the expertise of certified medical transcriptionists (CMTs), registered health information technicians (RHITs) and medical transcription instructors. The average MedGarde transcriptionist has 15 years experience.

Leveraging Technology

Beyond making sure we have the best qualified staff to transcribe your dictation, we also maximize the use of the latest technology. MedGarde’s transcription platform ensures document quality in three distinct ways:

Spelling:

Spell check is automatically run at the completion of each document so it is impossible for spell check to ever be skipped.

Grammar:

Grammar check is automatically run at the completion of each document so it is impossible for grammar check to ever be skipped.

Templates:

Formats are always consistent as our system employs templates.

Dictation Discrepancies

Any documents that contain any inaudible portions or other discrepancies are brought to the attention of the dictator, or other designate, and the issue is clearly explained. No guess work is made when it comes to ensuring that your documents are accurate and complete. Clients taking advantage of our electronic signature service, MedSign, have additional tools to communicate with our transcription staff regarding dictation issues.

Auditing

Clients wishing to audit the quality of their transcribed reports have access to the original dictated audio files via phone and online.

Demographics

While, dictators are responsible for providing patient name spellings and medical record numbers either during dictation or via other means, MedGarde offers flexible options for getting your demographic information to us. Options can be as simple as faxing your patient list to us to the more involved option of deploying an HL7 interface between your system and ours.

Full Audio Review

When beginning a new account, all work must pass both QA Level 1 and QA Level 2 until the transcriptionist has demonstrated proficiency with the account. After the transcriptionist has demonstrated proficiency with the account, full audio edits are conducted at the discretion of MedGarde and the client.

Reporting

Clients can elect to receive customized reports that show that the documents are being returned according to the agreed upon turnaround time.

Industry Guidelines

The standards of excellence set forth by the Association for Healthcare Documention Integrity (formerly the American Association for Medical Transcription) are followed unless otherwise specified by the client.
